I certify that I am at least 21 years of age. I understand that my Carfagna’s Wine Club Membership (“Membership”) renews automatically every (1) months for a total of twelve (12) automatic wine shipments (“Automatic Shipment”) per year. The variety and number of bottles I receive is determined by the Membership option I have selected. The cost of each Automatic Shipment is the value of the club and benefits alike (less my membership discount), plus tax. I authorize Carfagna’s Market to automatically bill my credit card on the first of each Automatic Shipment month (“Billing Date”) for this amount. I understand that I may cancel my Membership without penalty with 30 days written notice submitted to Tom@carfagnas.com and that my Membership will continue until cancelled. I can cancel by phone: (614) 846-6340, by email:Tom@carfagnas.com. To avoid being charged for an Automatic Shipment, a cancellation request must be submitted thirty (30) days before the next Billing Date. Automatic Shipments processed prior to a cancellation are nonrefundable, even if not yet received. 

We, Carfagna's Market, are willing to hold your wine club releases for 60 days. After 60 dayas we will not be able to guarantee availability of the wine. We will try our best to have comparable alternatives.
